What's the protocol for mixing an MAOI with a TCA? Is one supposed to get to a therapeutic dose of the TCA FIRST, then cautiously add the MAOI? Or is it the other way around?
Asking as I've been on 100mg+ of Nortriptyline for over a week and have yet to see even the tiniest bit of difference, and I'm thinking of keeping my current dosage of 200mg Nortripytline, and dumping my 100mg of Milnacipran i'm currently augmenting with, in favor of cautiously adding Parnate at 40mg.
Once the Milnacipran is out of my system, is it safe for me to start right away at Parnate 40mg / day with 200mg of Nortrip.?

The accepted protocol is to add the MAOI to an ongoing treatment with the tricyclic or begin them both at the same time. However, I have added the TCA to an ongoing treatment with an MAOI several times without incident. In any event, you would be doing it the right way according to traditional thought.

i think the tca is more directly controlled and should be added second
